Basic immunization is an effective preventive measure to protect infants from infectious diseases. At Sibuhuan Community Health Center, Complete Basic Immunization (IDL) coverage only reached 61% of the 75% target, with a sample compliance rate of 54.8% against the 100% ideal target. This study aims to determine the relationship between mothers' knowledge about basic immunization and compliance with basic immunization for infants in the working area of Sibuhuan Community Health Center, Padang Lawas Regency. This quantitative research method applied a cross-sectional design with a purposive sampling technique involving 93 respondents with infants under 24 months old. Data were collected using structured questionnaires and Health Alert Card (KMS) observation sheets, then analyzed univariately and bivariately using the Chi-Square test. The results showed that 54.8% of respondents were compliant and 45.2% were non-compliant, with the statistical test yielding a p-value = 0.032 (p < 0.05), proving a significant relationship. It is recommended that Sibuhuan Community Health Center optimize its educational and counseling programs intensively regarding the immunization schedule timeline to boost maternal compliance.
