Abstract

Preoperative phase require stage needed a structured, comprehensive, and documented risk identification system. Initial observations and interviews with the head nurse and nurses in the Central Surgical Unit of a hospital indicated that identification of surgical readiness risks had not been conducted systematically, while cases of surgical delay or cancellation had previously occurred. This community service activity aimed to optimize service quality and patient safety through the implementation of a web-based digital preoperative Early Warning System (EWS). The implementation method included observation, interviews, document review, development of the EWS instrument, socialization, demonstration, practice, implementation, and evaluation using an attitude questionnaire administered before and after the activity. The EWS consisted of eight parameters: patient readiness, human resources, equipment, instruments and CSSD, medications and medical consumables, infection prevention and control, communication and teamwork, and leadership and surgical risk management. The evaluation of 12 nurses following the implementation of the EWS, which involved orietation, education, and hands-on implementation, using eight attitude statements showed that the total score increased from 243 to 290, a difference of 47 points, or an overall increase in attitude of 19.34%. Seven of the eight indicators improved, while one indicator remained unchanged. The digital EWS-CSI implementation provided a more structured process for risk identification, documentation, interpretation, and follow-up. This program is expected to enhance nurses' readiness in the CSI, thereby strengthening patient safety
