Empowering Head Nurses in the Implementation of Patient-Focused Care through the Organization of Modified Team Nursing Care

Patient-focused care requires integrated care and collaboration, both intra- and interprofessional. Its implementation is often hampered by one of the understandings of the implementation of patient-focused care and the organization of nursing care that has not facilitated integrated care. This community service aims to improve nurses' understanding of the implementation of patient-focused care through organizing team care. The community service method is through empowering the head of the room and the nurse in charge of care/team leader. The results obtained were an average knowledge before education of 56.67 and knowledge after education of 97.67, an increase of 41 points. Based on the dependent t-test, the t-value was -9.689 and P = 0.000, so there was a significant difference between knowledge before education and knowledge after education in the head of the room and the nurse in charge of the patient/team leader. Regular education should be carried out on the implementation of patient-focused care and integrated care and the organization of modified teams should be implemented for all inpatient rooms.
